Nomination Form
The advancement document consists of a cover sheet and the documentation/evaluation
section. The documentation section is not to exceed 4 pages. See appendix
to the nomination form for examples of types of information desired
in sections 10c and 10d of the document.
Section 10A is the summary and evaluation of performance section. This
section includes:
- Brief overview of candidate’s qualifications/ achievements
- Evaluation by supervisor or department head; include name and title of the person who prepared the evaluation
- Early advancement justification (if applicable) signed by department
head
The Dean's Committee suggests candidates review the preliminary
evaluation form when completing documents.
Include information about the impact the candidate's work has had.
Documents should focus on what has happened since the last
advancement.
The Dean's Committee asks that nominees watch the font and margin
size. Documents with small type and margins are
difficult to read.
